Could needle deflection also be a factor going through multiple heavy layers ?

Like the needle bending because the tension is so tight? The way I had it set last it wasn’t bending the needle, but when I first adjusting it I noticed that if I maxed out the tension it would bend the needle as it came down. Is this what you’re referring to?

Yes and possibly making contact causing the issue with the thread
 
Like it was mentioned earlier I think you need to up your needle size.
I’m using a size 18 needle for Tera 40 (Tex 75)which is close to v69 thread right? I’ll have to see if I can go up to a 20 size needle on my machine. I really just need to find heavier duty machine.

Or the needle end slightly bending due to pressure of the material it is passing through…
 
I use a 18 with tera 40 - no problems at all.
I haven't had an issue with this set up until this point. Have you sewn thick material with this? I think I remember you said you did #18 needle tera 40 thread with your binding machine, but heavier thread and needle for your weight rated stuff. I read that I can go up to size 21 needle per the manual so I am going to pick up a pack just to try while I trouble shoot and fix or buy a new plate.

I actually used 18s for my v92 thread as well. Again, only had issues with dull needles. Also, with very thick sewing and deflection.
Ok, I'll see about drilling out that hole and sanding it down and giving it another go. That really makes me think its rubbing against that jagged old cover plate. Would making the hole too big present any other issues I am not foreseeing?

I read on sailrite that some users will use different size thread for their bobbin vs. their top thread. Is this a bad path to go down? Assuming that the threads are tensioned equally what issues would this present? Thoughts on this being worth it?

Stick with same size thread. You will run into issues by one thread pulling more than the other a lot of fiddling. Don’t ask how I know. I have set up several machines for different applications and I use 18 needles with the v69 and no problem and if I start to bind a little I hand wheel it and on my industrial I use 20 needles with v69 or 92. Which reminds me I need to finish setting up my servo motor for that machine I’m still using the motor on the consew but hopefully finish fully converting it sometime this summer. One thing I have found out that all v69 are not created equal. I have run into that issue with it breaking and fraying. I switch thread and now no problem

Keep with the same size thread on top and bottom, adjust tensioning properly and a size 20 or 21 needle with 92 thread.

With what machine though? Punching power and type of tensioner can effect that as well. My newer Juki has a double tensioning system. It is also really strong so I can get away with smaller eye needles, in my sail rite with single tensioning system I had to go one size up over normal recommendations (only when sewing heavy duty webbing not when doing grosgrain or thinner stuff)
 
Ok fellas I’m about to get back at it. I drill open the jagged hole to take away some of the rough spots and sanded down top and bottom. No break through! I then backed off the top and bobbin tension and tried 2 layers then 4 layers of nylon webbing without any tears or frays.
