A rope man probably would have prevented the fall because he had the Prussik tied incorrectly.
Complacency does kill. Were you in the service ?

Saddle assasin - google arborist 2in1 lanyard. There are different layouts but the concept is the same. 1 rope that you can work off and be tied off on either end.

I didn't have time to read everything since I’m at work but did a quick scroll through and this is my impression. I see a lot of badly tied prusiks(as well as badly spelled) and a lot of overhand knots. The rope in question looks like the standard Walmart special 300lb working load haul rope labelled not for climbing use or life support or something to that effect.

Guys please know your accessory cord as some of the new more supple rope requires a triple fisherman’s knot instead of the standard double due to issues with the core pulling through during shock loading. Also a proper prusik the knot should be on the biner side not around the wraps. Also to reiterate what I'm sure everyone has already said check your setup every time you go up.

What do you mean the know should be on the Biner side.

When I used a prusic I tied an over hand, but routed it like the black rope in post 27. The knot sees little if any force that way. I bought some of the sewn versions but dang are they long. I now use the ropeman1.

USMC, but that was 30 years ago. I have instructed motorcycle safety (teaching beginners how to ride) and a firearms instructor. All three push that complacency kills.

WRT the ropeman, I was wondering what would happen if he fell a certain distance until the line got compressed at the ropeman. Would the ropeman handle the fall? Would it cause damage to the tether? I believe the ropeman is not designed or tested to take a shock. But I could be wrong.
