Could be that he had untied it at some point and didn't re-tie it correctly.I’ve repeatedly put hundreds of pounds of force over probably a hundred hours on those HSS prusiks before I got the tether and LB I use now. There is ZERO way I can believe that prusik “came untied for unknown reasons”. Is it the safest option out there? Probably not. But you would have to intentionally alter the knot in some way to cause that problem. I honestly felt perfectly secure hanging from one of those, especially as a LB. I feel bad for this guy but I can not in my right mind or conscious heart believe that was the rope’s fault.

Yes. The F8OB is very tightly dressed and the tail end is shackled to the working line, then the tag end was knotted and shackled as well. The prusik is also knotted and shackled and very neatly dressed. They also mention only about 10K times in the instructions mine came with (dvd AND paper) that you should always check your prusik for dressing and 3 wraps on each side.Does it come with a stopper knot? Hum. I looked at these on line. Pretty sure you wouldn't catch me hanging from those knots they have on there. I bet over time they pushed down and came apart. I don't think they where intended to be continually loaded like he was using it. Maybe @Brocky can offer some knot in sight.
