Most people use "tether handles" of various designs(para cord loops, s binders, purpose designed 3d printed ones, whatever you have on hand can be made to work), editing this to add if you have a rappell rope and a push pole you could set your tether considerably higher and eliminate a few top tether movesdo people advance their tether with a small forked stick?
As stated below, most people use some form of loop, handle or webbing style keeper that allows them to pull up the rope without their fingers being under the pinch point of the tether. And as to your other question, I don’t think it will eliminate your leg strap from kicking out altogether. That all makes sense.
